I do believe RW and Twanger have hit the nail on the head and the powder weight/quality issue was my first thought on reading of your dilemna.
You might also try reducing those charges of loose to about 80 or 90 grains volumetric and see if the groups settle in.
And Mark suggested swabbing the bore after each shot - don't forget that step. Leaving that out could explain what you are seeing with the primers.
